Fiscal Year 2021 Open Enrollment

Due to the pandemic, the City of Malden will not be hosting a Benefits Fair, however we will still move forth with Open Enrollment. Open Enrollment is currently open for coverage effective July 1st, 2020. Deadline for paperwork is May 22nd, 2020.

MEDICAL INSURANCE INFORMATION:

City employees, Municipal and School, are offered the option of Harvard Pilgrim medical insurance HMO and PPO. To find out more about the two plans you can find the HMO Schedule of Benefits here and the PPO Schedule of Benefits here. You can find a list of covered Preventive Care here. Harvard Pilgrim offers Virtual Visits to all clients, which you can find out more about here

If you choose to enroll, fill out the Harvard Pilgrim Enrollment form and submit your completed form* to Crystal Morano at cmorano@cityofmalden.org or send a physical copy to: Human Resources Department, 17 Pleasant Street, 3rd Floor, Malden, MA 02148.

As of July, the City will be changing to bi-weekly pay. You can view the new Medical Premium Rates for Municipal Employees here and School Employees here.

DENTAL INSURANCE INFORMATION:

The City of Malden offers dental coverage through two different companies: Delta Dental and Dental Maintenance Services.

In the past Malden has offered two plans through Delta Dental: Delta Dental Table Premier and Delta Dental Premier PPO Plus. The Table Premier plan is only offered to existing members.  The PPO Plus plan has more comprehensive coverage than the previous Table Premier plan, you can find the PPO Plus plan’s overview here. Dental Maintenance Services (DMS) is an HMO dental plan, you can find their overview here and a list of the network dental offices here. If you choose to enroll with DMS, fill out the enrollment form here

FLEX SPENDING:

The City of Malden offers Flex Spending through the Cafeteria Plan Advisors. This benefit allows you to set aside a portion of your pay before taxes to cover out-of-pocket medical expenses. Deadline to enroll is June 12th, 2020.

HEALTH CARE: Includes co-pays (medical & prescription), deductible expenses, non-cosmetic dental work, orthodontics, prescription eyeglasses, contact lenses, laser eye surgery, alternative health therapies (e.g. acupuncture), mental health services, and MORE! Current Max. Annual Election: $2,750. 

DEPENDENT CARE: For children under under 13 and dependents with special needs. Eligible expenses include: day care, preschool, before & after school care, summer day camp, and elder day care. Current Max. Annual Election: $5,000.
